REQUEST FOR U-1A

Energy Weldfab Inc. charges a base price per serial number for each U-1A requested. Any U-1A issued 2016-Present will incur a base charge of $55. Anything issued prior to 2016 will incur an $105 base charge. Once payment is received, the U-1A will be sent out within 5-10 business days. Requests may be expedited for an additional charge of $100 and processed within 1-2 business days.
